3. Review the Sweat Equity Policy

4. Complete the Consent Form

5. Complete the Application Form

6. Forward the completed Application Form and Consent Form to Cabot Habitat

The screening and assessment process will involve reviewing the documents submitted and other means as determined by the Family Selection Committee. Interviews will be held with families who are screened into the process and accepted based on the documents received. Members of the Family Selection Committee will complete these interviews.

Upon completion, a recommendation will be made to the Cabot Habitat Board of Directors for final approval.
